We know that coping with a long term health condition such as cancer, diabetes or cardiovascular disease is hard enough, but it can feel almost impossible if you're struggling with your mental health too.
What we can do to help you
We’ve created the ‘Passport to My Health and Wellbeing' which is a way to help people along their journey and empower them to keep on top of important information.
It's a simple but effective document that helps people who are living with a long term health condition and poor mental health to describe what they need from health professionals.
It's also a diary for appointments, a record card for vital details and a log for thoughts and questions.
The Passport makes life easier for them and for family and friends supporting them on their cancer journey.
How can I get a Passport?
You can ask your GP, care team or any other healthcare professional for a Passport and mention us when you do.
I’m a GP, how can I share the Passport with my patients?
We're always looking to connect with other healthcare professionals, especially if you work in mental health, cancer, cardiovascular or diabetes care. Our Passport can be shared by GPs or other practitioners and is a free resource to be shared across the North East of England.
